A few things:
1) Please fill out the form completely. Answer all questions.
2) Please send a deposit check of $60 to me to hold your spot. If I do not receive the deposit I cannot register you.

3) Please print and fill out the code of conduct from the USFHA website. If you are a member of an international club please note that in the comments section at the end of the registration questions. Foreign members do not need a code of conduct.

4) Please e-mail or snail-mail a copy of your USFHA membership card. Log In to USFHA and select PRINT MEMBERSHIP CARD.
Use your mouse to HIGHLIGHT the card. Copy the card and paste it in Microsoft Word or in a similar program. This will copy the image so you can e-mail it or print it. I do not need the whole page that appears. This has your password and log in information. Please just copy the card and send it as a word attachment or print it to mail it to me.
5) Total costs will be around $170. USFHA has raised their costs... I will let everyone that registers know the final amount due when I know. Please send deposits by Sept 10, 2008.
